Compensation: $72,000 - $85,000 Per Year DOE
Overview: Nexus HR is looking for a compassionate and experienced Licensed Marriage and Family Therapist (LMFT) / Licensed Clinical Social Worker (LCSW) / Licensed Clinical Psychologist (LCP) / Licensed Professional Clinical Counselor (LPCC) to join our client’s primary care team in Westminster, CA.
About the Job
This role integrates behavioral health services within an outpatient primary care clinic setting, supporting patients with mental health, emotional, and family-related concerns. The clinical provider will work closely with primary care providers in a collaborative, team-based environment to deliver holistic, patient-centered care. This is a fast-paced setting with a strong focus on whole-person care. Bilingual proficiency in English and Vietnamese is required to effectively serve the patient population.
Duties and Responsibilities:
Provide individual, couples, and family therapy in an outpatient primary care setting
Conduct psychosocial assessments, diagnosis, and treatment planning
Deliver evidence-based interventions for conditions such as anxiety, depression, stress, trauma, and behavioral concerns
Collaborate with primary care providers (PCPs) and medical staff to coordinate integrated care
Participate in case conferences and care team meetings
Educate patients and families on coping strategies, behavioral health, and wellness
Maintain accurate and timely clinical documentation in the electronic health record (EHR)
Monitor patient progress and adjust treatment plans as needed
Provide crisis intervention and referrals to higher levels of care when appropriate
Ensure compliance with HIPAA, state regulations, and clinic policies
Qualifications:
Minimum 1–2 years of clinical experience (primary care or integrated setting preferred)
Master’s degree in Marriage and Family Therapy or related field
Active and unrestricted LMFT/LCSW/LCP/LPCC license in the State of California
Bilingual in English and Vietnamese (required)
Experience working with diverse patient populations
Strong understanding of behavioral health integration in medical settings
Experience with brief intervention models
Familiarity with Medi-Cal and managed care populations
Ability to work efficiently within short visit timeframes
Excellent communication and interpersonal skills
