Compensation:  $240,000 - $280,000

About the Job

Our Client is looking for a PACE Primary Care Provider for their team in Orange County, California. This is a great opportunity for a Physician who has recently completed their advanced practice degree as for established practitioners looking for a new position. This is an excellent practice for a provider to grow a long-term career and patient base.

Duties and Responsibilities:

  • Oversight of clinical care provided to PACE participants by Mid-Level Providers
  •  Complete annual Mid-Level Provider reviews 
  • Participates in provider meetings 
  • Ensure appropriate provider coding 
  • Coordinate appropriate PACE specialist network for PACE participants 
  • Participation in PACE QAPI program 
  • Participation in PACE quality initiatives 
  • Ensure appropriate and efficient clinical operation workflows 
  • Continually monitor, evaluate and adjust clinic workflows to ensure appropriate  efficiency including, but not limited to the following workflows: 
    1. Triage 
    2. Appointment scheduling 
    3. Abnormal labs 
    4. Medication refills 
    5. Missed appointments 
    6. Order reconciliation 
    7. Standing orders 
    8. Rooming of participants 
  • Ensure appropriate inpatient and nursing facility utilizations 
  • Ensure appropriate outpatient specialty utilization
  • Ensure proper transitions of care 
  • Ensure POLST is completed and followed   
  • Communicate regularly with hospitals, nursing facilities and urgent care facilities.
  • Communicate and educate outpatient specialists on the PACE model of care.
  •  Work with pharmacists and specialists to utilize generic medications whenever possible
  • Work with in-house specialists on frequency of visits and the transition of stable  participants back to PCP 
  • Work with pharmacy to utilize 340b pricing whenever possible 
  • Complex Case Management (CCM) 
  • Oversight of IDT Team

Qualifications:

  • Board certified
  • Enrolled in Medi-Cal
  • Prior experience in a professional capacity may be preferred.
  • Computer skills.
  • Analytical aptitude, problem-solving skills, and the ability to manage a busy workload under pressure.
  • Experience in overseeing and leading a team
