Compensation: $64 to $71 per hour DOE

About the Job
The ideal candidate will be responsible for evaluating patients for appropriateness for the Functional Restoration Program, designing individualized rehabilitation programs, able to work within the interdisciplinary team and independently, able to handle multiple roles. This role requires a strong clinical background, excellent communication skills, and a passion for helping people achieve their mobility goals.
Duties and Responsibilities:
Perform thorough physical assessments and evaluations of patients as part of the multidisciplinary evaluation for appropriateness to the Functional Restoration Program
Develop personalized treatment plans based on patient evaluations and needs.
Implement and monitor treatment interventions, minimizing manual treatments while educating patients to manage their pain independently.
Educate patients about the rehabilitation process, their injury and pain, acute vs chronic pain, home exercise programs, and flare up management strategies.
Work with individual patients as well as groups.
Lecture patients on a variety of physical topics related to chronic pain and biopsychosocial model
Collaborate with multidisciplinary team to ensure comprehensive patient care.
Track and document patient progress and document weekly.
Maintain up-to-date knowledge of physical therapy techniques and best practices.
Understand MTUS guidelines for Functional Restoration programs.
Qualifications:
Previous experience in a clinical or rehabilitation setting is preferred; experience in a chronic pain setting is strongly preferred.
Graduate of an accredited Doctor of Physical Therapy (DPT) program.
Current state licensure as a Physical Therapist (PT).
Strong interpersonal and communication skills.
Ability to work effectively both independently and as part of a team.
Excellent problem-solving skills and attention to detail.
Compassionate, patient-focused approach to care.
Complex Regional Pain Syndrome (CRPS) experience is a plus.
Understanding of the California Workers’ Compensation System is a plus.
Bilingual is a plus.
Able to work in San Jose, CA.
