Overview:  Nexus HR is seeking a full-time Family Physician for a reputable primary care facility located in Peoria, IL, which operates as a Federally Qualified Health Center (FQHC).

About the Job

We are looking for a dedicated and compassionate Family Physician to join our primary care team. In this role, you will provide personalized healthcare and comprehensive medical services, addressing a wide range of medical conditions. You will develop healthcare and wellness programs tailored to each patient’s unique medical history and needs.

Duties and Responsibilities:

  • Provide primary healthcare services to patients

  • Conduct initial assessments and diagnose patients

  • Promote health education, hygiene, and disease prevention

  • Maintain accurate patient records for diagnostics and referrals as necessary

  • Order and interpret laboratory tests and procedures to aid in diagnosis

  • Inform patients and relevant parties about procedures and test results

  • Develop personalized health plans based on patient medical histories

  • Monitor, adapt, and administer individualized healthcare plans

  • Utilize patient health data for medical analysis and conduct comprehensive wellness exams

  • Administer vaccinations as required

  • Educate patients and caregivers on health management and disease prevention strategies

  • Refer patients to specialists and recommend specific treatments or procedures

  • Stay updated on the latest medical advancements and practices

Qualifications:

  • 2 years experience as a Family Physician

  • Family Physician internship and residency

  • Board Certified by the American Board of Family Medicine

  • Licensed to practice in Illinois

  • Active participation in continued medical education and membership in a professional organization is highly preferred

  • Able to work in Peoria, IL
